Please bring the following requirements

(one photocopy each and present the original copy):

• Duly accomplished OFW Information Sheet (Form to be given by OWWA)
• Passport (page with personal data with signature; work visa, if any)
• Work Visa/Work Permit
• Employment Contract (latest; verified by POLO, if possible)
• Proof of active employment – any of the following:

1. Recent Payslip
2. Recent Certificate of Employment issued by employer

• Payment of US$25.00 (this is valid for 2 years membership)

III. Other services:

*For prospective employers who are interested in hiring workers from the Philippines for the first time, our POLO team would like to meet with you to discuss the recruitment and deployment process of the Philippine government. Please send us an email at laborofficewdc@gmail.com.

*POLO, through OWWA, can also provide trainings during the outreach missions. If our OFWs would like to request for a particular training/seminar, please submit your proposals to washington@owwa.gov.ph.
